Job summary

BlueCross BlueShield of Tennessee, Inc. is seeking a Support Coordinator for ECF Choices in Maury/Williamson County. You will coordinate health, behavioral health, and long-term supports, helping members achieve independent living and employment through holistic planning.

The role requires travel to members' homes within 35 miles, a valid driver's license, and RN or related degree with proper certifications. This is a full-time daytime role with no on-call requirements.

Qualifications

• RN with CDNN/DDC certification for RNs or DDC for LPNs, or a related BS.
• MSW or LCSW preferred; QDDP or QIDP eligibility is acceptable.
• 5+ years as a independent Support Coordinator or case manager.
• Experience in HCBS HCBS Waiver setting is valued.

Responsibilities

• Partner with members and families to identify supports and services.
• Collaborate with clinical and social support teams to meet needs.
• Conduct face-to-face visits to assess physical, behavioral, and living needs.
• Authorize home and community based services using criteria.
• Maintain a valid driver’s license and travel to homes as needed.
• Complete required immunizations or medical tests when needed.
• Complete digital literacy assessments as part of job duties.
