Job summary

Qualifications

• RN with relevant DD certifications or equivalent licensure.
• BS in social work, nursing, education, human service or health care field; LMSW/LCSW preferred.
• QDDP or QIDP certification; 5+ years case management for HCBS waiver.
• Experience in healthcare or operations environment (2+ years).

Responsibilities

• Partner with members and families to identify supports and services.
• Coordinate with clinical and social support teams to meet health and living needs.
• Conduct face-to-face visits to assess needs and status.
• Authorize home and community based services per criteria.
• Maintain driver license and vehicle policy compliance.
