Job summary

Lewis Mason Thurston Area Agency on Aging is seeking a qualified professional to assist functionally impaired adults in identifying, obtaining and utilizing services to preserve independence. The role requires conducting comprehensive assessments, developing individualized service plans, and coordinating with providers in a hybrid work setting.

The position involves reassessing client needs, authorizing services, and maintaining case records, while adhering to applicable rules and policies.

Qualifications

• Master’s degree in behavioral or health sciences and one year of paid social services experience.
• OR Bachelor’s degree in behavioral or health sciences and two years of paid on-the-job social services experience; OR Bachelor’s degree and four years of paid social services experience.

Responsibilities

• Completes comprehensive assessments via in-depth interviews and collateral contacts.
• Develops or modifies individualized service plans.
• Implements service plans with provider arrangements and supports.
• Performs reassessments to ensure plans meet client needs.
• Manages reauthorizations and authorizes services within guidelines.
• Maintains client records, data, and required reports.
