Job summary

Area Agency on Aging & Disabilities of SW WA seeks an experienced case manager to assess needs of adults with disabilities and older persons, authorize in-home and community services, and help clients remain independent in their communities. You will develop and administer service plans, coordinate with providers, and ensure timely service delivery while maintaining records.

Travel to client homes and community sites is required.

Qualifications

• Master’s degree in behavioral or health science with 1 year social service experience OR
• Bachelor’s degree in behavioral or health sciences with 2 years social service experience OR
• Equivalent mix of education and experience.
• Valid driver’s license and vehicle insurance; reliable transportation.

Responsibilities

• Perform comprehensive functional assessments and reassessments for clients.
• Develop a plan of care to meet identified outcomes and coordinate services.
• Implement short- and long-term service plans with in-home/community supports.
• Coordinate with providers, resolve service delivery concerns, and ensure records are updated.
• Travel to client homes and community sites; liaison with multiple agencies.
