Job summary

Independence Association seeks a full-time case manager to join our adult case management team. You will assess needs, plan services, advocate, and coordinate resources to help individuals with intellectual disabilities or autism live full, inclusive lives.

You will work with the person and their support network to develop yearly Person Centered Plans, document thoroughly, and participate in trainings and supervision. Travel with a valid driver’s license is required.

Qualifications

  • Bachelor's degree required.
  • 1 year experience as Adult Case Manager required.
  • Strong written and oral communication; personable; team player.
  • Comfortable advocating for individuals with various levels of disability.

Independence Association seeks a full-time case manager to join our growing adult case management team. As a case manager, you will play a key role in assisting individuals with intellectual disabilities or autism, by accessing resources to assist them to live a full and inclusive life. You will do this by providing thorough assessments of strengths and needs, comprehensive planning of service needs, advocacy, outreach and resource coordination unique to every person served. You will work closely with both the individual and his or her identified support group, and be a leader in assisting individuals to develop a yearly Person Centered Plan to achieve their goals. Requirements

Travel required (must have valid driver’s license)
