Qualifikationen

• Degree in a human service field.
• Ability to research and locate information, advice, guidance and support.
• Strong IT skills including managing websites and social media.
• Excellent listening and written communication.
• Knowledge of Greater Boston community support systems.
• Committed to the rights of seniors and disabled people.
• Flexible, reliable, and able to cope with emergencies.
• Prior home care experience is helpful.

Aufgaben

• Collaborate with care teams to support participants' goals.
• Provide resources to promote safety and independence.
• Assist training and supervision of caregivers and PCAs.
• Conduct home visits and document activities.
• Perform clerical and administrative duties.
• Provide health status information as needed.
• Complete required documentation promptly.
• Maintain a clean, insured vehicle for client transport.
• Follow service plan objectives.
• Adhere to service policies and procedures.
