Job summary

Aging &Long Term Care of Eastern Washington is seeking a Case Manager to provide advanced community-based case management for older and disabled adults in Spokane County, WA. The role involves home visits, assessments with CARE, care plan development, and coordination with DSHS and providers.

This non-exempt, full-time position requires a strong knowledge of aging services, proficiency with Word/Excel, and the ability to manage documentation, payments, and safety reporting.

Qualifications

• Master’s degree plus 1 year paid social service experience.
• Bachelor’s in behavioral/health sciences with 2 years paid experience.
• Bachelor’s plus 4 years paid social services experience.
• Two years of college courses in a relevant field and two years paid experience.
• Bilingual or bicultural candidates encouraged to apply.

Responsibilities

• Provide advanced case management for older/disabled adults under Title XIX.
• Develop and implement care service plans; coordinate with DSHS and providers.
• Ensure authorized payment for personal care services.
• Prepare case files and reports; maintain electronic records.
• Act as mandated reporter for abuse/neglect; coordinate investigations.
• Participate in Administrative Hearings when needed; may testify.
• Provide out-of-home placement case management as needed.
• Assist with advocacy, networking, and community resource liaison.
