Job summary

Area Agency on Aging & Disabilities of Southwest Washington seeks a skilled case management professional to assess clients, develop care plans, and coordinate in-home and community services across Clark, Cowlitz, Wahkiakum, Klickitat, and Skamania Counties.

The role focuses on helping older adults and adults with disabilities with compassionate, linguistically inclusive support. It requires travel, strong record-keeping, and collaboration with providers in a non-exempt, weekday schedule.

Qualifications

• Master’s degree in behavioral or health science with one year of on-the-job social service experience.
• Bachelor’s degree in behavioral or health sciences with two years of paid social service experience, or higher qualification.
• Possession of a valid driver’s license and minimum state-required vehicle insurance and dependable transportation.

Responsibilities

• Perform comprehensive functional assessments and reassessments for clients.
• Develop an appropriate plan of care to assist clients with identified outcome objectives.
• Implement short- and long-term service plans, including authorization of in-home and community services.
• Resolve client concerns with service delivery quality.
• Coordinate with providers of client services and follow-up on terminations.
• Utilize in-house nursing staff for case consultation and provider oversight.
• Maintain manual and computerized client records.
• Coordinate services with other community providers including mental health and disability services.

Education
