FeldCare Connects is seeking a passionate Physical Therapist to deliver quality home healthcare as an Independent Contractor in the Los Altos area. You will work with patients of all ages in their homes, determining and executing the plan of care with one-on-one treatment and seamless documentation in our EMR.

Qualificações

• Active Physical Therapist license issued by the state board.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone required.
• Bilingual a plus.

Responsabilidades

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report patient response to treatment and plan of care.
• Provide one-on-one treatment in patients' homes.
• Complete documentation in the EMR during or after visits.
