FeldCare Connects in California is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages as an independent contractor serving El Dorado Hills and surrounding areas.

You control your own schedule, work in your preferred coverage areas, and determine earnings based on per-visit patient load. We provide administrative support and a streamlined referral process via Cliniconnects to access patients quickly.

Qualifications

• Active Physical Therapist license in California.
• License must be current, active, and unrestricted.
• Travel to patient homes with reliable transportation.
• Smartphone required for scheduling and documentation.
• Bilingual abilities are a plus.

Responsabilités

• Provide one-on-one physical therapy treatment to patients at home.
• Assess patient progress and adjust plan of care.
• Document visits and outcomes in Cliniconnects app.
• Coordinate with care teams and scheduling.
