Job summary

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages as an Independent Contractor serving Azusa and the surrounding areas.

You control your own schedule and coverage areas with no minimum requirements, while benefiting from strong per-visit compensation and administrative support to coordinate care. You determine the plan of care and document patient progress using our EMR system.

Qualifications

• Ability and enthusiasm in working with all ages.
• Current Physical Therapy license issued by the state board of Physical Therapy.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapist program.
• Reliable personal transportation.
• Smartphone.
• Bilingual a plus!

Responsibilities

• Provide quality home healthcare to patients of all ages as an Independent Contractor.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care for each patient.
• Utilize the Cliniconnects app to accept or decline patients and coordinate care.
