Qualifications

• Ability and enthusiasm in working with all ages.
• Current Physical Therapist license issued by the state board of Physical Therapy; active and unrestricted.
• License must be current, active, and unrestricted; no conditional or restricted licenses.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone required for communications and documentation.
• Bilingual capabilities are a plus.

Responsibilities

• Deliver high-quality home healthcare to patients of all ages in Pacoima and surrounding areas.
• Observe, record, and report patient response to treatment; adjust plan of care as needed.
• Provide one-on-one treatment and determine the plan of care with autonomy.
• Utilize our EMR system for documentation during or after visits.
• Coordinate with FeldCare Connects team and local home health agencies.
