Qualifications

Responsibilities

• Develop, implement, and revise Case Manager treatment plans; monitor progress toward goals.
• Meet with patients weekly for Case Management.
• Complete all case management documentation per regulatory standards.
• Collect and document urinalysis and breathalyzer tests as required.
• Coordinate referrals to physicians and other professionals.
