Responsibilities

• Plan and coordinate activities to help patients meet coordination of care needs.
• Provide basic consultation with treatment team oversight and family/community coordination.
• Develop and implement a plan of care for post-hospitalization needs and continuity of care.
• Discharge planning including scheduling appointments and communicating with families.
