Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded at a partnered dialysis clinic in Oklahoma City. On-site role focused on coordinating patient care, follow-ups, and education with the clinical team.

You will engage patients in person, schedule provider visits, document care plans in the EMR, and communicate clearly with patients, families, and staff to maintain continuity of care in a fast-paced clinical setting.

Qualifications

• Legally authorized to work in the United States
• Active, unrestricted clinical certification or licensure (e.g., PCT, CNA, MA/CMA, LPN, RN)
• Dialysis-specific certification preferred (CDT, CHT, CCHT, CCHT-A)
• Experience coordinating patient appointments and managing provider schedules
• At least 1 year direct experience with hemodialysis/nephrology patients
• Excellent communication, organization, and bedside manner

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and transitions
• Coordinate care using Ennoble Care’s EMR to ensure continuity and follow-up
• Schedule patient visits and support daily clinic workflow
• Collaborate with providers and dialysis staff for integrated patient-centered care
• Serve as liaison between providers, patients, families, and staff
• Communicate patient needs and changes promptly to clinical leadership
• Maintain accurate documentation per standards and regulations
• Demonstrate professionalism and sound judgment in a fast-paced clinical environment
• Support team collaboration and use of Microsoft Word and Excel
