Job summary

Ennoble Care in Tulsa, OK is seeking a full-time Care Coordinator to join our office-based clinical care team. This role supports patient-centered care coordination, documentation, and timely follow-up across care settings.

The position works closely with providers within a family-practice-style model of care. Candidates must have active clinical licensure (MA/CMA, LPN, RN) and at least 1 year of relevant healthcare experience.

Qualifications

  • Legally authorized to work in the United States.
  • Active, unrestricted clinical certification or licensure (MA/CMA, LPN, RN).
  • Minimum 1 year of clinical and administrative or care coordination experience.
  • Proficient with Microsoft 365 apps and internal systems.
  • Strong organizational and communication skills; no on-call requirement.

Responsibilities

  • Coordinate patient-centered care activities using EMR system.
  • Support providers with documentation, follow-up tracking, and care coordination.
  • Serve as communication link between providers and care teams regarding updates and priorities.
  • Communicate patient needs and urgent concerns to the clinical team promptly.
  • Assist with referrals and care transitions with external partners.

Ennoble Care is seeking a full-time Care Coordinator to join our team in an office-based setting in downtown Tulsa, Oklahoma. This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care.

While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.

KEY RESPONSIBILITIES
  • Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system
  • Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination
  • Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner
  • Assist with care transitions, referrals, and coordination with external partners, as needed
  • Collaborate closely with fellow Care Coordinators and internal care team members within an office-based environment
  • Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operations
  • Contribute positively to team collaboration, workflow efficiency, and day-to-day office operations
REQUIREMENTS
  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure, such as:
  • Medical Assistant (MA) / Certified Medical Assistant (CMA)
  • Licensed Practical Nurse (LPN)
  • Registered Nurse (RN)
  • Minimum 1 year of experience in a clinical and administrative or care coordination role within a healthcare setting
  • Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and internal systems
  • Cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence
  • Well-developed organizational, time-management, and attention-to-detail skills, paired with an optimistic approach and initiative-driven mindset
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. CST, in an office-based setting, in the downtown Tulsa, Oklahoma area
  • No on‑call
  • Daytime hours
  • Seven (7) company‑paid holidays annually
  • Ability to pass a background check and drug screen

Pay Rate: $17.00 - $18.00
