Qualifications

• High School diploma or equivalent required; college degree preferred.
• 1–2 years of recent PSC experience in home health or hospice setting preferred.
• Excellent written and spoken communication; problem-solving skills.
• Proficiency with scheduling software and EMR systems; 40 WPM typing and accurate data entry.

Responsibilities

• Schedule all visits for field staff during the home health episode.
• Coordinate admissions and timing with caregivers and patients; ensure timely admission.
• Maintain daily/weekly reports for Clinical Manager.
