Job summary

Creative Solutions in Healthcare, Inc. is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house team on a PRN basis. You will provide person-centered care to help residents improve mobility, strength, and quality of life under supervision of a Physical Therapist.

The role requires an active Texas PTA license and prior SNF or long-term care experience is preferred. Net Health documentation experience is a plus, and applicants enjoy flexible PRN scheduling and collaborative

Qualifications

• Must hold an active Texas PTA license.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.

Responsibilities

• Administer medically prescribed physical therapy treatments.
• Provide skilled physical therapy services under PT supervision.
• Relate information and referrals to the Physical Therapist.
• Implement and modify plans of care under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient response and adjust approaches with the team.
• Provide education to patients, family, and caregivers.
• Ensure thorough and timely documentation per company policies and regulations.
• Attend required meetings with the Director of Rehab.
