Job summary

Creative Solutions In HealthCare is seeking a compassionate PRN Physical Therapy Assistant (PTA) to join our in-house rehab team. You will provide person-centered care to help residents improve mobility, strength, and overall quality of life under the supervision of a Physical Therapist.

This role offers flexible PRN schedules in a skilled nursing setting. Texas license must be active; SNF or long-term care experience is preferred.

Qualifications

• Active Texas PTA license required in good standing.
• Experience in SNF or long-term care preferred.
• Knowledge of physical therapy treatment practices.
• Ability to adapt plans under PT supervision.
• Accurate documentation and communication with care team.

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain and improve function.
• Provide PT services under supervision of a PT.
• Respond to service requests and relay information to PT.
• Modify care plans under PT supervision.
• Measure motor function and document progress.
• Collaborate with the interdisciplinary team to adjust interventions.
• Educate patients, families, and caregivers.
• Ensure timely, compliant documentation per policies.
• Attend rehab department meetings.
