Job summary

Creative Solutions in Healthcare, Inc. in Texas is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis. You will deliver person-centered care to help residents improve mobility and quality of life under PT supervision.

This role requires a valid Texas PTA license, familiarity with SNF or long-term care, and ability to use Net Health. We value strong communication, teamwork, and flexible scheduling.

Qualifications

• Active and valid Texas PTA license.
• SNF or long-term care experience preferred.
• Knowledge of Net Health EDS is a plus.

Responsibilities

• Administer prescribed physical therapy treatments under PT supervision.
• Provide skilled PT services and interventions.
• Relay information and referrals to the PT.
• Implement and modify care plans under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and adjust interventions with the team.
• Provide education to patients, family, and caregivers.
• Ensure timely, compliant documentation.
• Attend meetings as designated by Director of Rehab.
