Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Jellico, TN. This PRN role supports therapy services under the supervision of a licensed Physical Therapist, focusing on resident mobility, strength, balance, and functional goals.

The position requires an Associate’s degree in physical therapy, active PTA licensure, and a commitment to ongoing professional development.

Qualifications

• Associate’s degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license required in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying targets, goals, and treatment plans.
• Conduct individual and group treatment as outlined in the treatment plan, including strength, mobility, gait, balance, ROM, and modalities.
• Assist in selecting PT techniques/media and sequence of activities; adapt as indicated.
• Monitor residents’ response to intervention and consult with the PT as needed.
• Establish and modify resident schedules with the PT and team members.
• Adhere to department and resident schedules; modify as appropriate for treatment regimens.
• Train residents, family, and staff in use of equipment.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Report to work on time and coordinate schedules for productivity.
• Assist with Quality Assurance and documentation in CMS-compliant EMR systems.
