• Active Physical Therapy license in state(s) of practice.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents to develop treatment plans with short and long-term goals.
• Develop and implement treatment plans and communicate findings to residents and families.
• Complete CMS-compliant documentation in NetHealth EMR.
