Evolve Therapy Services in Sevierville, TN is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility to support resident rehabilitation under the supervision of a Physical Therapist. Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to continuing education to maintain licensure and certification.
• Documentation in CMS/NetHealth EMR as required.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in evaluations under supervision.
• Assist in establishing treatment goals and plans.
• Conduct individual and group treatments as outlined in the treatment plan (strength, mobility, gait, ROM, balance, etc.).
• Assist in selecting techniques/media and adapt as indicated by residents.
• Monitor residents’ response to intervention and consult with PT for modifications.
• Assist in scheduling with the Physical Therapist and other team members.
• Adhere to department and resident schedule; modify as needed for treatment regimens.
• Train residents, families and staff in use of equipment.
• Participate in resident, family and staff education.
• Maintain sound judgment in evaluation, planning, implementation and follow-up of therapy programs.
• Report to work on time and coordinate schedules for productivity.
• Assist with Quality Assurance programs and CMS documentation in NetHealth EMR.
