Job summary

Evolve Therapy Services in Tampa, FL, is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role supports therapists, screens residents, and helps develop treatment plans within a team. Various shifts may be available including full time, part time, and PRN.

Responsibilities include direct patient care, group and individual sessions, and documentation in NetHealth EMR. A Florida PTA license and ongoing CE credits are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluation of residents under the Physical Therapist's direction.
• Help identify short-term and long-term goals and treatment plans.
• Carry out individual and group treatments focusing on strength, mobility, balance, and gait.
• Assist in selecting techniques/media and adapting them per resident performance.
• Monitor resident responses and report modifications to the Physical Therapist.
• Support scheduling in coordination with the Physical Therapist and team.
• Adhere to resident schedules and treatment regimens; adjust as needed.
• Provide training to residents and families in equipment use.
• Participate in resident, family, and staff education.
• Ensure timely, compliant documentation in CMS and NetHealth EMR systems.
