Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility, CHAPEL HILL NURSING CENTER, RANDALLSTOWN, MARYLAND. This full/part-time/PRN role emphasizes quality resident care under supervision and ensures CMS-compliant documentation.

The PTA will participate in screenings, evaluations, goal setting, and both individual and group therapy, while collaborating with the Physical Therapist and care team.

Qualifications

• Associates Degree in Physical Therapy from an accredited program.
• Active state Physical Therapy Assistant license.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist in evaluation and planning under the Physical Therapist's direction.
• Develop and follow treatment plans, goals, and modalities as appropriate.
• Educate residents, families and staff on therapy activities and equipment.
• Maintain compliant documentation in NetHealth EMR per CMS expectations.
