Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Richmond, VA. This PRN position supports the Physical Therapist in delivering daily therapy services and documentation in a CMS-compliant EMR.

Responsibilities include screenings, treatment planning support, and discharge planning in collaboration with the therapy team. Requires an Associate's degree and an active PTA license in the state; ongoing education is encouraged.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in developing treatment plans with short- and long-term goals.
• Deliver individual and group treatment on mobility, strength, balance, gait, ROM, and related modalities.
• Monitor resident responses and consult with Physical Therapist for modifications.
• Assist in scheduling and coordinating resident treatment with the team.
• Train residents and families in equipment use and safety.
• Participate in resident, family, and staff education.
• Maintain compliant and medically necessary documentation in NetHealth EMR.

Education

Associate's degree in physical therapy from an accredited program
