Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Durham, NC. This full-time, part-time, or PRN position focuses on planning, directing, and delivering high-quality PT services to residents, in compliance with CMS and state regulations.

You will conduct screenings, develop treatment plans, and collaborate with families and the care team to achieve resident goals, with emphasis on documentation, billing, and patient-centered care.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Perform any other duties as assigned to support resident care.
