Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Durham, NC. This in-house role offers Full Time, Part Time, or PRN options and focuses on delivering high-quality therapy to residents.

The role requires a Bachelor of Science in Physical Therapy, an active license in the state(s) of practice, and ongoing professional development. Competitive pay and benefits are available to eligible full-time staff.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Screen residents regularly to determine need for intervention.
• Screen residents referred to therapy to determine need for intervention and treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and recommendations to residents, families, and the inter-disciplinary team.
• Complete CMS-compliant and medically necessary documentation in NetHealth EMR systems.
