Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, TN, under the supervision of a Physical Therapist.

Responsibilities include resident screenings, evaluating needs, developing treatment goals, and delivering individual and group therapy. The role emphasizes education for residents, families, and staff, with a focus on quality care and adherence to CMS guidelines.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and develop target goals and treatment plans with the PT.
• Conduct individual and group treatments as outlined in the plan of care.
• Assist in selecting techniques/media and adapt activities based on resident progress.
• Monitor resident responses and modify as indicated in collaboration with the PT.
• Assist in establishing and adjusting resident schedules with the care team.
• Train residents, families and staff in equipment use and precautions.
