Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Jellico, TN. This PRN role supports licensed Physical Therapists, conducts screenings, and contributes to individualized treatment plans while maintaining CMS-compliant documentation in NetHealth EMR.

The ideal candidate holds an Associate's degree in physical therapy, an active PTA license in the state, and commits to ongoing professional development.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the Physical Therapist.
• Assist in establishing target lists, short-/long-term goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan, including strength, mobility, gait, ROM and modalities.
• Assist in selecting techniques/media and adapt them as indicated by resident performance.
• Monitor resident response and consult with the Physical Therapist for modifications.
• Assist in scheduling with the Physical Therapist and team members.
• Adhere to schedules and modify as needed for treatment regimens.
• Train residents, families and staff in equipment use.
• Participate in resident, family and staff education.
• Demonstrate sound judgment in planning and follow-up of therapy programs.
• Report to work on time and coordinate schedules for productivity.
• Assist with Quality Assurance Program and CMS documentation in NetHealth EMR.
