Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility partner in Kingsport, TN. The role supports the Physical Therapist in delivering resident-centered care.

Responsibilities include screenings, treatment planning with goals, individual and group sessions, documentation in NetHealth EMR, and participation in education and QA activities. Requires an Associate's degree and an active PTA license with ongoing professional

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in the evaluation under the direction of the Physical Therapist and in identifying a treatment plan.
• Assist in establishing and modifying resident schedules in conjunction with the Physical Therapist and other team members.
• Conduct individual and group treatment focusing on strength, mobility, balance, gait, ROM, and modalities as outlined in the treatment plan.
• Monitor residents' responses to intervention and report modifications to the Physical Therapist.
• Document compliant and medically necessary therapy services in NetHealth EMR.”
• Participate in resident, family, and staff education and QA activities.
