Evolve Therapy Services, LLC is hiring a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Antioch, TN. The PTA will work under the supervision of a Physical Therapist to deliver skilled therapy services to residents.

Responsibilities include screenings, contributing to evaluations, implementing treatment plans, and documenting progress in the CMS-aligned NetHealth EMR system. The role supports resident mobility and functional goals in a team-based setting.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in developing target goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatment as outlined in the plan including mobility, gait, ROM, strengthening, and modalities.
