Job summary

Evolve Therapy Services seeks a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Antioch, TN. This full-time role collaborates with a Physical Therapist to implement and monitor treatment plans, deliver group and individual therapies, and ensure CMS-compliant documentation in NetHealth EMR.

Qualifications

Associate degree in physical therapy from an accredited program.
Active PTA license in state(s) of practice.
Commitment to continuing education and licensure maintenance.

Responsibilities

Provide physical therapy services under supervision of a Physical Therapist.
Conduct screenings to determine intervention needs.
Assist in evaluations and treatment planning with the Physical Therapist.
Support scheduling and coordination of resident therapies.
Educate residents and families on therapy plans and equipment usage.
Document compliant and medically necessary CMS-related records in NetHealth EMR.
