Job summary
Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Antioch, TN to support the Physical Therapist, conduct screenings, and assist in treatment planning.
The role offers Full Time, Part Time or PRN options and requires an Associate's degree and an active PTA license in Tennessee; CMS/NetHealth documentation is part of daily duties.
Qualifications
- Must hold an Associate's degree in Physical Therapy.
- Active Physical Therapy Assistant license in state(s) of practice.
- Ongoing continuing education to maintain licensure and certification.
- Experience with resident care planning and documentation is a plus.
Responsibilities
- Provide physical therapy services to residents under the supervision of the Physical Therapist.
- Conduct screenings of residents to determine intervention needs.
- Assist in evaluations and treatment planning under PT guidance.
- Develop target and treatment goals with the PT and care team.
- Deliver individual and group treatment focusing on mobility, strength, balance, and ROM.
- Assist in selecting techniques/media and adapt as needed.
- Monitor resident response and consult PT for modifications.
- Help establish resident schedules and coordinate with the team.
- Ensure timely, compliant documentation in NetHealth EMR.
