Job summary

Evolve Therapy Services is hiring compassionate Physical Therapists for an in-house Skilled Nursing Facility located in Pahokee, FL. Positions are available as Full Time, Part Time, or PRN to support resident care and program implementation.

Responsibilities include evaluating residents, creating and adjusting therapy plans, and ensuring CMS-compliant documentation in NetHealth EMR. A BS in Physical Therapy and active licensure are required, with ongoing professional development encouraged.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure/certification.
• Assume responsibility for ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards of practice and department procedures.
• Adhere to documentation and billing procedures.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Complete CMS-compliant and medically necessary documentation in NetHealth EMR.
• Complete other duties as assigned to support resident care.
