Evolve Therapy Services is seeking a Physical Therapist to provide in-house skilled nursing facility therapy at our partner location in Pahokee, FL. The role supports full-time, part-time, or PRN schedules, focusing on resident-centered care and team collaboration.

The ideal candidate holds a BS in Physical Therapy, an active state license, and a commitment to ongoing professional development to ensure high-quality therapy services and compliant documentation.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services following standards of practice and department procedures.
• Document and bill PT services according to CMS expectations.
• Evaluate residents to develop treatment plans and goals.
• Develop and adjust treatment plans with short- and long-term goals.
• Screen residents to identify need for intervention or treatment.
• Communicate evaluation findings and plans to residents, families, and the care team.
• Complete compliant documentation in NetHealth EMR system.
• Discharge or transition residents as appropriate and in the best interests of the organization.
