Evolve Therapy Services is seeking a licensed Physical Therapist to join our partner facility in Pahokee, FL, an in-house Skilled Nursing Facility. Full-time, part-time, or PRN positions available with a $5,000 sign-on bonus.

Responsibilities include evaluating residents, developing treatment plans, documenting care in NetHealth EMR, and collaborating with the interdisciplinary team. Qualifications include a BS in Physical Therapy and active license; ongoing CEUs required; competitive pay and

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Provide physical therapy services to residents per established standards and procedures.
• Document and bill therapies according to procedures.
• Develop treatment plans with short and long-term goals.
• Evaluate residents to gather data for treatment planning.
• Communicate findings and recommendations to residents and families, and to the interdisciplinary team.
• Complete CMS-mandated documentation in NetHealth EMR.
