Job summary

Evolve Therapy Services, LLC in Pahokee, FL is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility. This full-time, part-time, or PRN position focuses on delivering high-quality therapy services to residents and collaborating with the interdisciplinary team.

Responsibilities include planning and directing PT services, documenting per CMS guidelines, and conducting regular screenings to determine treatment needs.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services in accordance with standards and procedures.
• Document and bill therapy services per company policies.
• Evaluate and plan resident therapy programs with interdisciplinary team.
• Screen residents regularly to determine intervention needs.
• Coordinate evaluation findings and treatment recommendations with residents and families.
• Develop and implement short- and long-term treatment goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Perform duties to support resident care and organizational interests.
