Job summary

Evolve Therapy Services in Pahokee, FL is seeking a Physical Therapist for an in-house Skilled Nursing Facility. The role supports planning, directing PT services, and delivering resident-centered care in accordance with CMS expectations.

Responsibilities include evaluating residents, developing treatment plans, documenting care in NetHealth EMR, and collaborating with an interdisciplinary team. Requires BS in Physical Therapy and active state license; CE to maintain licensure.

Qualifications

• Minimum ofBachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
