Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility located in Le Center, MN. The position is Part Time or PRN.

The therapist will plan, develop, and direct Physical Therapy services to maintain quality resident care in line with CMS expectations and facility procedures. Responsibilities include evaluating residents, developing treatment plans, documenting in NetHealth EMR, and coordinating with families and the interdisciplinary team.

Qualifications

• Compassionate care required to support residents.
• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and productivity targets.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents and develop treatment plans with short and long-term goals.
• Screen residents to determine intervention needs and plan treatments.
• Communicate evaluation findings and plans to residents and interdisciplinary team.
