Evolve Therapy Services in Holly Hill, FL is hiring a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role supports the Physical Therapist with screenings, treatments, and CMS documentation across full-time, part-time, and PRN opportunities.

Ideal candidates hold an Associate’s degree in physical therapy, an active PTA license, and commit to ongoing education to maintain licensure. You will collaborate with the care team to maximize resident outcomes.

Qualifications

• Active Physical Therapy Assistant license in state(s) of practice.
• Associate's degree in physical therapy from an accredited program.
• Ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the Physical Therapist's direction.
• Assist in establishing and modifying resident treatment plans and goals.
• Deliver individual and group treatment focusing on strength, mobility, balance, gait, and ROM.
• Assist in selecting PT techniques/media and adapt as indicated.
• Monitor residents’ response and report modifications to the PT.
• Maintain scheduling in coordination with the PT and care team.
• Complete compliant CMS documentation in NetHealth EMR system.
