Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Worthington, MN. The role requires working under the supervision of a Physical Therapist across full-time, part-time, or PRN schedules.

Responsibilities include conducting screenings, contributing to evaluation and treatment plans, and delivering both individual and group therapy focusing on mobility, strength, gait, and range of motion.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans, including short- and long-term objectives.
• Deliver individual and group treatment focusing on strength, mobility, balance, gait, and ROM.
• Assist in selecting techniques/media and adapting activities per resident performance.
• Monitor resident responses and consult PT for modifications as needed.
• Assist in scheduling in collaboration with the PT and team members.
• Ensure adherence to resident schedules and treatment plans.
• Educate residents, families, and staff on therapy devices and applications.
• Maintain documentation to CMS expectations in NetHealth EMR.

Education

Associate's degree in Physical Therapy

Active PTA license in state(s) of practice

Continuing education to maintain licensure
