Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Florida. You will work under a Physical Therapist to deliver inpatient therapy services, perform screenings, and contribute to the progression of individualized treatment plans.

Responsibilities include conducting group and individual sessions, tracking outcomes, and coordinating with the care team while maintaining CMS-compliant documentation.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in treatment planning under the Physical Therapist guidance.
• Assist the Physical Therapist in establishing short- and long-term goals and treatment plans.
• Conduct individual and group treatments in areas such as strength, mobility, balance, gait, ROM and modalities.
• Assist in selecting PT techniques/media and adapt as indicated by resident performance.
• Monitor progress and report necessary modifications to the Physical Therapist.
• Assist in scheduling in coordination with the PT and team members.
• Adhere to schedules and modify as needed for treatment regimens.
• Train residents and staff in equipment use and safety.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license
