Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Ahoskie, NC. The role supports residents with comprehensive PT services, following standard practices and CMS guidelines, in a collaborative care setting.

Responsibilities include assessment, treatment planning, documentation, and communication with families and the interdisciplinary team. Licensure, ongoing CE, and a passion for resident-centered care are essential.

Qualifications

• Must have active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services according to established standards and procedures.
• Document and bill services in accordance with company procedures.
• Evaluate residents and develop short and long-term treatment goals.
• Communicate findings and plans to residents, families, and the care team.
• Complete CMS-compliant documentation in NetHealth EMR.
