Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Ahoskie, NC. The role supports full-time, part-time, or PRN schedules in a compassionate, patient-centered setting.

Responsibilities include evaluating residents, planning interventions, and coordinating with an interdisciplinary team. Active licensure and ongoing CE are required to maintain licensure and provide high-quality care.

Qualifications

• OT degree required.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents to develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with the team.
• Conduct therapy according to the treatment plan and document and bill services.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement compliance.
