Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Ahoskie, NC. Full Time, Part Time, or PRN positions are available to provide comprehensive OT services within a facility setting.

Responsibilities include screening, evaluation, treatment planning, scheduling, documentation, and coordination with the care team. Must hold an active OT license in the practicing state and pursue ongoing education.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments follow reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education to maintain licensure
