Job summary

Evolve Therapy Services in Ahoskie, NC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility. Full Time, Part Time, or PRN positions available.

You will plan, organize, develop, and direct OT services, screen residents, develop treatment plans, and collaborate with families and the interdisciplinary team.

Active OT license required; continuing education encouraged; benefits include medical, dental, vision, and growth opportunities.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state.
• Degree in Occupational Therapy and current licensure are required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to identify best intervention for each patient.
• Evaluate residents to inform treatment plans and establish goals.
• Interpret and communicate findings to residents, families, and the team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy per plan with proper documentation and billing.
• Perform all other duties to effectively treat patients.
• Oversee COTA as needed and ensure proper documentation and reimbursement.
