Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Selma, Alabama. The role supports full-time, part-time, or PRN schedules to ensure quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures.
• Conduct evaluations and develop short/long-term treatment plans.
• Interpret and communicate evaluation findings and recommendations to residents and interdisciplinary team.
• Develop and implement treatment plans to meet identified goals.
• Ensure compliance with CMS expectations in NetHealth EMR systems.

Education
