Evolve Therapy Services is seeking a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Selma, AL. Join a team delivering high-quality patient care and performing comprehensive evaluations, treatment planning, and documentation in a CMS-guided environment.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and productivity targets.
• Document and bill therapy services per company procedures.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
