Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Selma, AL. You will work under the supervision of a Physical Therapist to deliver resident-centered therapy services.

The role involves screenings, treatment planning, and both individual and group sessions, with documentation in NetHealth EMR and participation in quality assurance.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying a target list, including short-term goals, long-term goals and treatment plan.
• Conduct both individual and group treatment as assigned by the Physical Therapist and outlined in treatment plan that includes muscle strength, coordination, mobility, sensory awareness, sitting and standing, transfer training, balance activities, gait training, ROM activities and modalities.
• Assist in selecting physical therapy techniques/media and sequence of activities, adapt techniques/media as indicated by resident performance after consultation with PT.
• Monitor residents’ response to intervention and consult with PT if modification is indicated.
• Assist in establishing and modifying individual resident schedules with PT and team members.
• Adhere to department and resident schedule and modify as appropriate for treatment regimen.
• Train resident, family and appropriate staff in the use or application of such equipment.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Report to work on time and coordinate schedule for maximum productivity and efficiency.
• Assist with Quality Assurance Program as required.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
